Rollover crash caused by ducks crossing I-95

PORT ORANGE, Fla. – Officials say a Brinks truck swerved and rolled over while trying to avoid a driver who had stopped on Interstate 95 to let a mama duck and her ducklings cross the road.

The Florida Highway Patrol says the accident happened Wednesday afternoon near the State Road 421 exit in Port Orange.

FHP says the car drove away before authorities arrived at the scene.

The people inside the armored truck received minor injuries that did not require a trip to the hospital. But because of the truck’s design, they needed help from the emergency crews to get out of the vehicle.

FHP says the mother was killed, but her ducklings survived. Volusia County Animal Control took the ducklings.

Four injured in SUV rollover on Florida’s I-75 near I-10

Four members of a Plant City family were injured Monday afternoon when a tire on their sport utility vehicle blew out, causing the vehicle to overturn on Interstate 75.

Trooper M.E. Birchard said the driver lost control of the 2002 Chevy Blazer when the left rear tire blew out. The Blazer rolled over several times, ejecting three of the four occupants. None of those in the Blazer was wearing a seat belt, according to Birchard.

The Blazer came to a stop with its roof on the median guardrail.

The driver, Jason Norris, 33, and his front-seat passenger, Tyler Norris, 13, were critically injured and were taken to Shands at the University of Florida. One of the back seat passengers, Hope Norris, 14, was taken to Shands in serious condition.

The other back seat passenger, Walker Norris, 7, was treated at Lake City Medical Center for minor injuries.

Birchard said alcohol was not a factor in the crash and charges are pending further investigation.

Port Orange man dies in motorcycle accident Sunday in Daytona Beach

The Florida Highway Patrol says a Port Orange man died in an accident in Daytona Beach Sunday afternoon. Sgt. Kim Montes says 56-year-old William Lee Hughes was northbound on his motorcycle on Tomoka Farms Road at International Speedway Boulevard, when a westbound car allegedly ran the traffic light at International Speedway Boulevard. Troopers say Hughes’ motorcycle struck the left-front fender of the car, driven by 31-year old Lakecia S. Harris of Deland, and Hughes was thrown from the motorcycle. The Florida Highway Patrol says alcohol was not a factor in the accident, and that the driver of the car received minor injuries. Troopers also say charges are pending.

The accident closed west-bound International Speedway Boulevard at Tomoka Farms Road Sunday afternoon while troopers conducted their investigation. Traffic coming from Daytona Beach on Speedway was detoured north on Tomoka Farms Road by Daytona Beach Police during the accident investigation, and traffic southbound on Tomoka Farms Road was turned around or detoured through the McDonald’s parking lot just before International Speedway Boulevard.

Florida officer hospitalized after crash

SOUTHWEST MIAMI-DADE, Fla. (WSVN) — Officials said an police officer was injured after a 3-car crash Wednesday morning.

Police said the three cars collided on the Shula Expressway, just before Florida’s Turnpike around 3 a.m. Paramedics transported a Miami-Dade police officer to the Jackson Memorial Hospital in stable condition.

Bus Company Suspended After i-95 Crash

Federal regulators have ordered Sky Express, the discount bus company involved in a fatal crash in Virginia this week, to halt its interstate bus service because of concerns about the carrier’s safety record, officials said Wednesday.

An accident involving a Sky Express bus traveling overnight from Greensboro, N.C., to New York’s Chinatown on Tuesday morning killed four passengers and injured dozens of others after the driver veered off Interstate 95 and the bus rolled over. The accident occurred in Caroline County, about 30 miles north of Richmond.

The authorities said driver fatigue played a role in the crash, and the driver, Kin Yiu Cheung, 37, of Queens, was arrested and charged with reckless driving. He suffered minor injuries.

The decision to suspend Sky Express came after the Federal Motor Carrier Safety Administration, which oversees bus safety, issued the company an unsatisfactory safety rating after conducting a compliance review.

The agency, which is part of the federal Department of Transportation, said it had found multiple violations in Sky Express’s driver qualification requirements, its drug and alcohol compliance measures, its hours of operation and its vehicle maintenance.

“Safety is our No. 1 priority,” Ray LaHood, the transportation secretary, said in a statement. “We will use every resource at our disposal to pursue and remove from our roads unsafe, reckless bus companies.”

Sky Express, based in Charlotte, N.C., has routes from New York to about a dozen cities in North Carolina, Florida, Georgia and South Carolina. It charges one-way fares of about $30. The company also operates in the District of Columbia and Virginia.

Sky Express, which did not return a phone call Wednesday, has had a poor safety record during the past two years, according to the Department of Transportation. The carrier has been cited 46 times by the Federal Motor Carrier Safety Administration since May 2009 for using fatigued drivers. It has also had four crashes.

Tuesday’s accident was at least the third fatal crash involving one of the many discount tour bus companies that use Chinatown as a hub.

In March, a World Wide Tours bus driver returning to Chinatown from a Connecticut casino swerved off Interstate 95 in the Bronx, slicing the bus in half and killing 15 people. Two days later, a Super Luxury Tours bus headed from Chinatown to Philadelphia crashed on the New Jersey Turnpike, killing two people, including the driver.

The Federal Motor Carrier Safety Administration said Wednesday that it had responded to the accidents by increasing enforcement, including conducting more than 3,000 surprise inspections of passenger buses during the first two weeks of May, which led to the suspensions of 127 drivers and 315 unsafe buses being taken out of service,

In New York, lawmakers have called for stricter regulations of the tour bus industry, including requiring carriers to apply to the city for permits in addition to the operating licenses they are required to obtain from the federal government.

Man faces DUI charge after leaving scene of injury car accident

A man was arrested for driving under the influence after he left the scene of an injury accident late Tuesday on U.S. 166 just west of the West Madison Bridge, a Cowley County Sheriff’s Department official said.

Arnold D. Snow, 49, posted bond after being charged by law enforcement officers with DUI, leaving the scene of an injury accident and failure to report an accident, said Undersheriff Bill Mueller.

Snow left on foot after the accident and was given a ride into Ark City, Mueller said. The Police Department located him at his residence and he was questioned by a sheriff’s deputy and given a sobriety test.

The two-vehicle accident was weather-related, Mueller said. The two vehicles did not collide but both slid into a ditch.

On-duty immigration agent involved in triple-fatal auto accident

Three women were killed and two children injured when an on-duty Immigration and Customs Enforcement agent reportedly ran a stop sign at Heber and Bowker Roads four miles east of Heber Tuesday evening, according to a Highway Patrol accident report.

The highway Patrol said the accident occurred at 7:10 p.m. when the northbound ICE agent “failed to stop for the posted stop sign” at Bowker and ran into the Dodge mini-van in which the women and children were riding.

Imperial County Coroner Sgt. Charles Lucas identified the three women killed as Imperial resident, Sandra Garcia, 41, who was driving the mini-van, Maria Consuelo Nieto, 49, of El Centro, and 47-year-old Patricia Martinez Reyes of Mexicali.

The two children, ages 11 and one, were properly belted into their seats, according to the CHP report, and were taken to El Centro Regional Medical Center.

The 36-year-old ICE agent, whose name has not yet been released, is being treated at Pioneers Memorial Hospital in Brawley. A statement released Wednesday morning by the ICE said:

“U.S. Immigration and Customs Enforcement (ICE) extends its heartfelt condolences to those affected by this tragic accident. Our thoughts go out to the families and loved ones of all those involved. ICE is working directly with the law enforcement agencies responsible for investigating the accident. The on-duty ICE agent who was involved is being treated at a local hospital and is expected to recover from his injuries.”

 The Insurance Institute for Highway Safety reports that in crashes involving large trucks and other vehicles, 98% of the fatalities occur to the people in passenger vehicles.

 The National Highway Traffic Safety Administration (NHTSA) has warned groups using 15-seat passenger vans about the dangers of fully loading the vans and then putting an inexperienced driver behind the wheel. The NHTSA also stated that 15-passenger vans are large trucks and should only be driven by people with experience driving large trucks.

 The FMCSA’s Motor Carrier Management Information System (MCMIS) classifies a truck as large if its gross vehicle weight rating (GVWR) exceeds 10,000 pounds.

 The majority of fatal crashes involving tractor-trailer trucks include trucks that are pulling one trailer. A recent survey showed that 64% of fatal truck crashes had one trailer. Thirty-two percent of those involved single-unit trucks (no trailer) and fewer than 4% of the those involved multi-trailer vehicles (more than one trailer).
